A guide for young people, parents and carers who are in the transition process of moving from childhood into adult life.

Description

At Barnsley Council, we have a new team in Adult Social Care to support young people who may need help from us when they turn 18. Our friendly team of social workers will support you in your transition and help you prepare for adulthood.

This is done in different ways depending on your age but may include:

  • Attending key meetings, such as Education, Health and Care plan reviews
  • Helping you complete a social care assessment
  • Linking you with local services that can help you achieve your goals.

The Preparing for Adulthood Process can help you if:

  • You're known to Children's Social Care, they will speak to you about your transition into adulthood and make a referral for you into our PFA team with your consent.
  • If you not known to Children's Social Care and you're under 18, You can contact their team to request an assessment of your needs. You can do this yourself, or you can get help from your parents, carers or a professional working with you. 
  • If you aged between 18 and 25, you can contact Adult Social Care to request an assessment of your needs. You can do this yourself or you can get help from your parents, carers or a professional working with you.
