SOCIAL CHANGE - NHS SOUTH YORKSHIRE INTEGRATED CARE BOARD (ICB) - TYPE 2 DIABETES SURVEY

Do you live in South Yorkshire, are over 16 years of age, and have Type 2 Diabetes? Share your experience on the best way to promote information relating to Type 2 Diabetes and be in with a chance of winning £100.

NHS South Yorkshire Integrated Care Board (ICB) launched a survey to understand the experiences of those with Type 2 Diabetes with using diabetes support services in South Yorkshire.

Your input is very valuable and will help us guide our efforts in improving the way support for people living with Type 2 Diabetes is promoted and delivered. The survey will only around 10 to 15 minutes of your time, and your responses will remain confidential.

Everyone that completes the survey will have the opportunity to enter a prize draw for a chance to win £100 worth of shopping vouchers.
