Description

The home based (domiciliary) physiotherapy service aims to provide a high-quality service for adults over 16 years of age within their own home. The service provides assessment and treatment to housebound patients, residents in care homes and patients with complex needs affecting access to clinics.

We accept referrals for a variety of musculoskeletal, neurological and respiratory conditions, falls and mobility problems. We provide a full comprehensive holistic assessment, individualised exercise programmes, and pain management. Additional specialist assessments are completed depending on the patient’s symptoms e.g. falls, neurological, chest assessments

We also run respiratory clinics, a drop in mobility clinic and a self-referral telephone triage clinic for mobility equipment.

Eligibility: Referral criteria: - Over the age of 16 - Patient must live within the Barnsley borough or be registered with a Barnsley GP - Housebound or has complex health needs and not able to attend a clinic or outpatient services. - Patient consent to assessment and treatment criteria Method of referral: - Anyone can refer for assessment for mobility problems - All other conditions require a referral form completing and forwarding to the physical health single point of access (SPA). Exclusions Patients should not be referred: - If they are aged less than 16 years old - If they are able to attend an appropriate outpatient clinic, unless for respiratory problems - If they have an out of area GP - For the supply of wheelchair only (patients must be already receiving physiotherapy and not be referred solely for wheelchair assessment)
