Social prescribers provide and source local support that's individually tailored to your health and wellbeing needs.

My Best Life is a social prescribing service for Barnsley, funded by Barnsley Clinical Commissioning Group.

Social prescribing is a means of enabling professionals to refer people to a range of local, non-clinical services. Recognising that people’s health is determined primarily by a range of social, economic and environmental factors, social prescribing seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.

My Best Life supports people to:

  • Lead healthier lifestyles
  • Improve the home environment
  • Increase community connections
  • Address money management and welfare benefit issues
  • Connect to work and volunteering

How can My Best Life support you?

Our Advisors work with people to help them identify and achieve their personal goals around health and wellbeing.

They’ll work with individuals for up to 12 weeks, to put together an action plan that can help with ‘what matters to you’. The plan is built around the Five Ways to Wellbeing. This is the New Economic Foundation’s evidence-based framework of actions that help people to lead happy fulfilling lives.

 Partnership working

My Best Life work closely with partners in the Health, Social Care and Voluntary, Community and Enterprise sector. We can support customers to access services and work together to achieve the best outcomes for customers.


Referrals can be made by any Primary Healthcare professional in Barnsley, including your GP or Nurse. You must be aged 18 or over to be referred.

Eligibility Criteria

  • Low level mental health
  • Socially isolated or lonely
  • Frequent attender
  • One or more managed health condition

My Best Life - Secondary Care

Our Secondary Care Team support customers referred via Secondary Care routes, such as the Emergency Department and Yorkshire Ambulance Service. This dedicated team of advisors will support customers who may be attending hospital regularly and may benefit from some social intervention.
