Making Plans is for your health and care in the future; for when you are older; for when you're nearer the end of my life.

Many people never think about making plans for their future health and care choices or making plans to say what should happen when they get older or when they are nearer the end of their life.

Making Plans for Your Future will tell you why these decisions and choices are important for you and help you with your Plans.
